(80) Can a Disposable Arthroscope Be Used to Effectively and Efficiently Perform Advanced Arthroscopy?

Abstract:

Background: Temporomandibular joint (TMJ) arthroscopy is a minimally invasive technique for diagnosing and treating TMJ disorders. However, the initial capital investment to perform arthroscopy can be cost prohibitive. Recently, disposable arthroscopes with Chip-on-Tip technology have gained traction, offering improved optics, ease of use and portability, along with a lower initial price point.

Purpose: This study explores the benefits, limitations, and efficacy of disposable “Chip-on-Tip” arthroscopes in TMJ arthroscopy.
Study Design and

Methods: Retrospective review of patients undergoing arthroscopy with a disposable arthroscope from August 2023 to December 2024. Data was collected on the complexity of procedure (Level one: diagnostic arthroscopy; Level two: advanced debridement; Level three: disc reconstruction), disease classification and procedure duration (minutes). Major complications including bleeding, otologic and intracranial injury were evaluated.

Results: 67 patients with 91 joints underwent arthroscopy with the disposable arthroscope. Average age was 41.0 years (14.4-90). Disease classification included 14 arthralgias, 71 articular disc disorders, 3 dislocations, and 5 pathology cases. Complexity of procedures included 5 Level one, 39 Level two, and 47 Level three. Average procedure duration was 73.9 minutes. No instrument breakage or major complications encountered during procedures.
Conclusion and Relevance: Chip-on-tip technology affords disposable arthroscopes strong optics and durability, enabling efficient performance in complex TMJ arthroscopy. Their portability, ergonomic design, and accessibility make them a viable alternative to reusable glass scopes. Future research should focus on improving functionality and assessing long-term cost-effectiveness to solidify their role as a sustainable option in TMJ arthroscopy. Advantages include eliminating sterilization processes, reducing infection risk, and ensuring consistent optical quality, with lower initial investment benefiting budget-constrained practices. However, challenges include increased medical waste, environmental concerns, higher per-procedure costs in high-volume centers, and limited features compared to high-end reusable systems. Future research should focus on improving functionality, minimizing environmental impact, and assessing long-term cost-effectiveness to solidify their role as a sustainable option in TMJ arthroscopy.
