Obstructive sleep apnea (OSA) is linked to several medical comorbidities. While continuous positive airway pressure (CPAP) is effective in alleviating airway obstruction, over 50 percent of OSA patients refuse or inconsistently use the mask. Surgical management of sleep apnea has significantly evolved over the last decade, especially with the introduction of the multilevel surgery concept. Drug-induced sleep endoscopy (DISE) has become pivotal in surgical planning. Recently, the hypoglossal nerve stimulator has emerged as a promising therapy for OSA. This session will discuss a multidisciplinary team approach to managing OSA as well as the placement and activation of the hypoglossal nerve stimulator. The session aims to explain the entire journey for patients with hypoglossal nerve stimulators from diagnosis to activation and the subsequent device adjustment, if indicated. Long-term outcomes after activating the device also will be discussed.

Learning Objectives:

At the conclusion of this presentation, participants should be able to:
  1. Examine the concept of nerve stimulation in the treatment of OSA.
  2. Explain the rule of drug-induced sleep endoscopy (DISE), validity, reliability, best anesthetic technique and results.
  3. Discuss the perioperative workup, indications and contraindications of the hypoglossal nerve stimulator.
  4. Describe the surgical procedure for the insertion of the hypoglossal nerve stimulator therapy and how to minimize complications.
  5. Identify the role of the sleep medicine team in activating the implant, titrating the therapy and managing persistent OSA.
