Surgical advancement of the maxilla can produce significant improvement in airway volume. However, there is still hesitation among many surgeons to advance the maxilla to a degree that is large enough to make a significant change due to esthetic limitations or long-term stability factors. Surgeons at the speaker’s institution perform over 300 maxillary-mandibular advancements a year and have learned some pearls and pitfalls regarding anticipating a large (over 10 to 15 mm) advancement in the maxilla.
