(61) Oral Aspergillosis in Post-COVID-19: A Clinical Confrontation in a Post-Pandemic Era

Abstract:
Statement of the Problem: Oral aspergillosis has emerged as a significant opportunistic infection in post-COVID-19 patients, posing a major clinical challenge in the post-pandemic era. Secondary immunosuppression from SARS-CoV-2, prolonged corticosteroid use, and pre-existing comorbidities increase the risk of invasive fungal infections, including oral aspergillosis. This condition can present with necrotic ulcerations, severe pain, and, in extreme cases, widespread dissemination. The lack of standardized diagnostic criteria, along with clinical variations, complicates early detection and timely treatment.
Materials and

Methods: A systematic review was conducted on studies published between 2020 and 2024 focusing on post-COVID-19 oral aspergillosis, including case reports, systematic reviews, and original research containing data on risk factors, clinical manifestations, diagnosis, and treatment. Excluded studies were those without confirmed diagnoses, lacking full-text access, or addressing non-oral aspergillosis. Databases such as PubMed, SciELO, and MEDLINE were searched to identify relevant articles. Data on risk factors, clinical presentation, diagnostic tools, and therapeutic approaches were extracted and synthesized.
Methods of Data Analysis: The selected studies were analyzed to identify commonalities among patients diagnosed with oral aspergillosis following COVID-19 infection. Key variables, such as patient demographics, comorbidities, corticosteroid use, clinical presentation, and treatment outcomes, were evaluated. A total of approximately 250 patients diagnosed with oral aspergillosis post-COVID-19 were included, with cases reported between January 2020 and March 2024. Descriptive statistics, including prevalence rates, mean percentages, and confidence intervals, were applied. Chi-square tests and logistic regression models were used to determine correlations between risk factors and disease progression. Additionally, a subjective analysis was performed due to the heterogeneity of case reports, allowing for a qualitative assessment to identify common clinical patterns and treatment responses.

Results: Review studies indicated that 23.3% of post-COVID-19 ICU patients had Aspergillus spp. coinfection, and the incidence of oral aspergillosis increased by 15% among immunosuppressed individuals. Common symptoms included necrotic ulcers (87%), pain (72%), and tissue necrosis (45%). Diagnosis relies on clinical assessment, histopathology, and microbial cultures, with galactomannan detection showing 70-80% sensitivity. Antifungal treatments like voriconazole and itraconazole were effective in 78-85% of cases, while 30-40% of severe cases required surgery. Mortality rates ranged from 36% to 50%, emphasizing the importance of early treatment.
Outcomes Data: Post-COVID-19 oral aspergillosis remains a diagnostic and therapeutic challenge due to delayed recognition and the absence of standardized management protocols. The association between prolonged corticosteroid therapy and increased fungal susceptibility underscores the need for careful monitoring of high-risk patients. Additionally, variations in individual immune responses to antifungal therapy highlight the importance of a personalized, multidisciplinary approach. Further research is needed to establish evidence-based clinical guidelines and effective preventive strategies.

Conclusion: Oral aspergillosis in post-COVID-19 patients represents an emerging and critical challenge in clinical practice. Its increased prevalence, particularly among immunosuppressed individuals, and its severe clinical manifestations underscore the need for early detection and prompt intervention. Despite advances in antifungal treatments, the high mortality rate highlights the need for standardized diagnostic protocols and more effective preventive strategies. A multidisciplinary approach, involving collaboration between medical, dental, and microbiological specialists, is essential to optimize patient outcomes. Future research should focus on developing evidence-based guidelines and exploring personalized treatment regimens to address the evolving nature of this infection in the post-pandemic era.
